Birth Certificate needed even if a passport is submitted
Both parents present with valid, government-issued picture ID, OR one parent present with valid, government-issued picture ID and a notarized letter from the absent parent giving written permission, along with a copy of the consenting parent's ID. ( ID not mandatory )
$25 application fee (cash, check or money order payable to: Sterling Municipal Library)
$80 passport book ( 4-6 week delivery ) ( Check or money order payable to: US Dept. of State* )
$15 passport card ( 4-6 week delivery ) ( Check or money order payable to: US Dept. of State* )
$60 expedite fee ( added for 2-4 week delivery ) ( Check or money order payable to: US Dept. of State* )
$14.96 (overnight return fee)

* Passport fee, expedite fee and overnight fee should be combined into one check or money order per application.
